Abstract:Myogenic tinnitus is a heterogeneous spectrum of tinnitus generated by abnormal muscle activity or modulated by musculoskeletal somatosensory input involving the middle ear muscles, velopharyngeal and peritubal muscles, facial nerve-related muscles, or cervicofacial myofascial structures. Rhythmic or clicking tinnitus is often broadly attributed to middle ear myoclonus, which may obscure site-specific differences in innervation, sound generation, triggers, and therapeutic targets. This review distinguishes direct myogenic sound generation from muscle-related somatosensory modulation and summarizes stapedius-related tinnitus, tensor tympani-related tinnitus, combined middle ear myoclonus, palatal tremor, peritubal and nasopharyngeal myogenic sound sources, facial nerve-related tinnitus, and cervicofacial somatosensory modulation. A mechanism-oriented diagnostic and therapeutic framework is proposed to support clinical localization and individualized management.
