日间加速康复外科模式下声带息肉切除术的疗效分析

Efficacy analysis of vocal cord polypectomy under daycare ERAS mode

摘要:

目的 分析日间加速康复外科(ERAS)模式下声带息肉切除术的临床疗效，并总结临床应用经验。方法 分析2023年6月—2025年6月在昆山市中医医院确诊为声带息肉并行手术治疗的78例患者的资料，术后随访4周。术后对患者的嗓音障碍指数(VHI)、住院时间、费用及并发症情况进行观察，用SPSS 27.0软件进行统计学分析。结果 78例患者经电子喉镜声带息肉切除术，术后2例发生恶心呕吐，1例发生黏膜水肿，术后4周复查VHI各维度评分较术前显著改善(P<0.01)，嗓音恢复率96.15%，住院费用为(3 143.67±434.36)元。结论 日间ERAS模式用于声带息肉切除术安全、有效，康复快，其成功实施依赖于精细化的操作、严格的患者筛选、围术期管理及术后随访。该模式具有显著的临床效益与社会经济价值，值得推广应用。

Abstract:

Objective To analyze the clinical efficacy of vocal cord polypectomy under the daycare enhanced recovery after surgery (ERAS) model and summarize clinical application experience. Methods An analysis was conducted on the data of 78 patients diagnosed with vocal cord polyps and undergoing surgical treatment at Kunshan Hospital of Traditional Chinese Medicine from June 2023 to June 2025. Postoperative follow-up was conducted for 4 weeks. Postoperative observations included the voice handicap index (VHI), length of hospital stay, costs, and complications. Statistical analysis was performed using SPSS 27.0 software. Results A total of 78 patients underwent electronic laryngoscopic polypectomy. Postoperatively, two patients experienced nausea and vomiting, while one of them developed mucosal edema. The VHI scores across all dimensions showed significant improvement at 4 weeks postoperatively,which was compared to preoperative levels (P<0.01). The voice recovery rate was 96.15%, and the hospitalization cost was (3 143.67±434.36) yuan. Conclusions The daycare ERAS model is safe, effective, and promotes rapid recovery for vocal cord polypectomy. Its successful implementation relies on meticulous surgical techniques, strict patient selection, comprehensive perioperative management, and postoperative follow-up. This model demonstrates significant clinical benefits and socioeconomic value, making it worthy of promotion.
