前臂游离皮瓣和股前外侧皮瓣在头颈恶性肿瘤切除术后缺损修复中的应用

【】 目的:比较前臂游离皮瓣与股前外侧皮瓣修复头颈部鳞状细胞癌术后缺损患者的恢复情况。方法:选取2019年1月到2022年12月在本科室治疗的40例头颈部鳞癌术后软组织缺损患者,随机行前臂游离皮瓣或股前外侧皮瓣修复。对比2组皮瓣成活率、并发症发生率及其它手术相关情况。结果:2组皮瓣成活率,皮瓣制备时间,手术时间、ICU留观时间、术后住院时间的差别无统计学意义(P>0.05)；2组并发症的发生率,前臂组明显多于股前外组,差别有统计学意义。结论:前臂游离皮瓣和股前外侧皮瓣均有良好的皮瓣存活率,是修复头颈部鳞状细胞癌术后软组织缺损的首选。临床使用中各有优势,需根据具体情况选择。

Abstract:

Objective To compare the repair effect of free forearm flap(RFFF) and anter -olateral femoral flap(ALTF)in postoperative defects after ablation of head and neck squamous cell carcinoma. Methods From January 2019 to December 2022 ,40cases of head and neck defects after radical ablation of head and neck squamous cell car- cinoma were randomly divided into 2 groups,one was repaired by RFFF and the oth -er by ALTF .The flap survival rate,operation time,complications were compared． Results There was no significant difference ( P>0.05) in the flap survival rate,flap making time,operation time,ICU stay,hospital stay between 2 groups.The incidence of post -operative complications in RFFF was higher than that in the ALFT group .Conclusion Both RFFF and ALTF have good flap survival rate,which are prior choices for soft tissue defect after ablation of head and neck squamous cell carcinoma.
