慢性鼻窦炎伴鼻息肉患者组织嗜酸性粒细胞与血清特异性IgE的相关性

摘要:

目的 分析慢性鼻窦炎伴鼻息肉(CRSwNP)患者组织嗜酸性粒细胞与血清特异性免疫球蛋白E(sIgE)的关系及其临床特征。方法 选取2019年6月—2021年6月行功能性鼻窦内镜手术的192例CRSwNP患者，根据血清sIgE检测结果分为sIgE阳性组和sIgE阴性组，比较两组患者组织嗜酸性粒细胞计数、术前VAS评分、Lund-Mackay评分及Lund-Kennedy 评分。结果 两组在性别、年龄比较无统计学意义(P＞0.05)；sIgE阳性与否和嗜酸性粒细胞是否浸润在例数上比较差异无统计学意义(P＞0.05)；在临床学特征方面，sIgE阳性嗜酸性粒细胞浸润组和其他3组比较在术前VAS评分(P<0.05)，Lund-Mackay评分及Lund-Kennedy评分差异具有统计学意义(P<0.05)。结论 CRSwNP患者组织嗜酸性粒细胞浸润程度与过敏反应关系并不确定。患者嗜酸性粒细胞浸润同时合并sIgE阳性主客观症状加重。

Abstract:

Objective To analyze the relationship between tissue eosinophils and serum specific immunoglobulin E (sIgE) in patients suffering from chronic rhinosinusitis with nasal polyps (CRSwNP) and their clinical characteristics.Methods A total of 192 patients with CRSwNP undergoing functional endoscopic sinus surgery from June 2019 to June 2021 were selected and divided into sIgE-positive group and sIgE-negative group based on the results of serum sIgE monitoring. The preoperative visual analogue scale (VAS) score, tissue eosinophil count, Lund-Mackay score, and Lund-Kennedy score were compared between the two groups.Results The differences of gender, age between the two groups were statistically insignificant (P＞0.05). There was no statistical difference between the number of cases with positive sIgE and eosinophils infiltration (P＞0.05). In terms of clinical characteristics, there were significant differences in preoperative VAS score, Lund-Mackay score and Lund-Kennedy score between sIgE-positive with eosinophils infiltration group and the other three groups.Conclusions There is no definite correlation between the infiltration degree of eosinophil and allergic reaction in patients with CRSwNP. The subjective and objective symptoms were aggravated in CRSwNP patients when eosinophils infiltration coexisted with positive sIgE.
