鼻颅底危重症患者护理体会（附1例报道）

摘要：目的探讨鼻颅底恶性肿瘤术后危重症患者的护理体会。方法回顾1例鼻颅底腺样囊性癌术后合并急性脑梗死患者的临床资料，根据患者病情制定鼻颅底危重症医护一体化护理模式和指导康复训练。结果经过医护13 d的个体化精细治疗与护理，患者颅底术区愈合良好，急性脑梗死症状基本缓解，肢体活动基本自如，转入康复治疗。结论鼻颅底恶性肿瘤病变范围大，解剖位置复杂，手术切除及修复难度大，患者术后潜在并发症多，通过对手术患者及时准确的观察及评估，进行早期预警识别干预，可以有效地减少或者避免患者术后并发症、严重后遗症的发生，保证患者顺利进入康复期。

Abstract:

ObjectiveTo explore the nursing experience of critical patients after surgery of malignancy in nasal skull base.MethodsClinical data of a patient with acute cerebral infarction after surgery of adenoid cystic carcinoma in nasal skull base were analyzed retrospectively. According to the condition, an integrated nursing care model with rehabilitation guidance for the critical patient was formulated.ResultsAfter 13 days of personalized meticulous treatment and nursing, the surgical region in the skull base healed well, the symptoms of acute cerebral infarction were almost relieved with free movement of limbs, and the patient was transferred to rehabilitation treatment.ConclusionMalignancy of the nasal skull base has a wide range of lesions, complex anatomical location, and difficulty in surgical resection and repair. There are many potential postoperative complications and acute cerebral infarction is one of the serious complications. Accurate observation and evaluation, early warning and identification as well as intervention can effectively reduce or avoid the occurrence of postoperative complications and serious sequelae, and therefore ensure the patients to enter the rehabilitation period smoothly.
